• Certification Details
• ACLS - Advanced Cardiac Life Support certification required. Must be from AHA or Canadian Heart & Stroke Foundation. If UD is not linked, defer to Clinical Tracks. Not required for Non-Clinical or remote candidates.
• BLS - Basic Life Support certification required. Must be from AHA or Canadian Heart & Stroke Foundation. If UD is not linked, defer to Clinical Tracks. Not required for Non-Clinical or remote candidates.
• PALS - Pediatric Advanced Life Support certification required. Must be from AHA or Canadian Heart & Stroke Foundation. If UD is not linked, defer to Clinical Tracks. Not required for Non-Clinical or remote candidates.
• NRP - Neonatal Resuscitation Procedure certification required. Must be from AAP, AHA, or Canadian Paediatric Society. If UD is not linked, defer to Clinical Tracks. Not required for Non-Clinical or remote candidates.
• CHEMO - Chemotherapy certification required. Must be verified via UD or Clinical Tracks. Not required for Non-Clinical or remote candidates.
• EFM - Electronic Fetal Monitoring certification required. AWHONN and NCC-EFM certifications are acceptable. If candidate has NCC-EFM but UD requires AWHONN specifically, confirmation is needed whether NCC-EFM can be accepted based on the contractual agreement.
• Tech Cert Verification - National certification required for Allied Professionals. Not required for Non-Clinical candidates.
• State License 1 Verification required for Allied Professionals.
• NURSYS Verification (RN, LPN/LVN, AP) required.
• IV Certification (LVN/LPN/Other) required. Must be verified via UD or Clinical Tracks.
• Management of Assaultive Behavior (MAB) certification required. Must be verified via UD or Clinical Tracks.
• NIH Stroke Certification required. Must be audited to ensure correct completion.
• Certification (Miscellaneous) required if specified in a UD.
• CPI - Crisis Prevention Institute certification required. Must be verified via UD or Clinical Tracks.
• Medication Test/Pharmacology Competency Exam required as applicable by Profession.
• Color Blind Testing Online competency required unit-by-unit basis as requested.
• Competency Exam required as applicable by Profession/Specialty.
• Competency Exam 2 (Medication Aide) required for CNAs in certain facilities.
• Photo (for Badge) required for certain locations. Passport style badge photos needed. Email photos to provided contacts or upload to the SharePoint Drive.
• Providence - Oregon Neglect Abuse Consent Form required only for candidates going to Providence Child Center facility.
• Providence - Workramp Modules must be completed by the candidate in Aya's LMS platform based on region and unit.
• Providence CA/Oregon - WorkRamp Core Path Modules must be assigned by the CS.
