*APPLICANT CLOSING DATE: Until Filled*STATUS: Full-time, Permanent FLSA: ExemptPOSITION TERM: Full-TimeREPORTS TO: Housing DirectorSUPERVISES: N/AWORKING HOURS:The wrap-around Care Coordinator is an exempt grant-funded position that provides case management services to adults and youth who require housing resources often due to unmet social, health, and behavioral health needs and may have criminogenic requirements. This position has regular business hours but usually involves work in the early mornings, evenings, and weekends. May be required to perform after-hours standby services as needed or some evening work, especially during the winter shelter season.SUMMARY:The wrap-around Care Coordinator is responsible for providing intensive care coordination for individuals, families, and youth who have been identified as high risk for homelessness or are currently homeless. The Care Coordinator will identify system partners such as Indian Health Services, Behavioral Health, Probation / Parole, Courts, Elder & Disability Services, Veteran Services, and Child Welfare to bring them together as Wrap Around Team to help empower individuals and families toward self-sufficiency and independence. The position assists and aids individuals and families requiring assistance from the Tribe. The position is central to linking homeless individuals and families to an appropriate housing plan of care and supporting them in tenancy-sustaining services. The goal is to facilitate rapid and resourced exits from homelessness into permanent supportive housing and provides participant advocacy, case management, benefits establishment, linkage to Mental Health and/or Substance Use Services, linkage to stable housing, and all other supportive services as needed.ESSENTIAL DUTIES AND RESPONSIBILITIES:Maintains a caseload of 12-15 individuals or families with a minimum of 14 hours of service contact per month per client, including weekly face-to-face communication with the client while promoting tribal wrap-around philosophy and practices.Assembles a Wrap Around Team within two (2) weeks of referral by interviewing the client and identifying family members/natural supports/agency representatives and other significant persons. Uncovering the client's strengths and needs helps with immediate needs and completes the initial Crisis Plan. Within the first 30 days, coordinates the meeting of the Team and develops the initial individualized Plan of Care (POC) based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan. The Plan should fit best with the client's culture, values, and beliefs. Monitor the provision and quality of services provided to the client and family through the Team and is the liaison when new services/resources need to be sought or developed. Provides or arranges transportation for their clients to appointments, crisis/emergency services, etc., if needed. Provides or secures support and crisis/emergency services for the client. Completes all the necessary paperwork in a strength-based manner and maintains accurate information online, ensuring that demographic data is also up to date. Collaborates with other system partners such as Indian Health Services, Behavioral Health, Probation / Parole, Courts, Elder & Disability Services, Veteran Services, and Child Welfare to bring them together as tribal wrap-around Team to help empower the client toward self-sufficiency and independence.Shall dress respectfully and maintain a respectful demeanor and presentation at all times. Provide written and verbal information related to the client's behavioral and mental health based on assessment and contact. Provide a comprehensive community-based safety plan with innovative strategies to assist the client in being maintained safely in the least restrictive setting. Collaborates and partners with the Rehousing Specialist or other staff. Submits reports to the Housing Director on a timely basis.Assists in the development and expansion of the Housing Department Programs as directed.Meets with the Housing Director for discussions and clarification on various issues as necessary.Attends all required meetings and training. Performs other duties as assigned.SUPERVISORY RESPONSIBILITIES:There are no direct reports for supervisory responsibilities for this position, but the position does require oversight, guidance, and direction for direct service providers.QUALIFICATIONS:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below represent the knowledge, skill, and ability required. A Master's Degree in Social Welfare (MSW) or a related field (Sociology, Criminal Justice, Human Services) is required. Five years of work experience in a position that required determining the eligibility of applicants to receive benefits may be substituted for the MSW DegreePrevious experience as a social worker, case manager, or care coordinator in a public or private agency is preferred.Two (2) years of prior experience working with at-risk individuals is required.Ability to manage time effectively.Ability to communicate effectively in writing or orally. Must be able to work independently and productively with minimal supervision.Must be able to get along with co-workers and work as a team.Must present a professional appearance at all times.Must maintain a high degree of confidentiality concerning all matters about the Tribe.Should possess extensive knowledge and experience working with Native American communities, culture, customs, and cultural law.TRIBAL AND INDIAN PREFERENCEUnder the Indian Self-Determination and Education Assistance Act (25 USC - 450e (b)), the Dry Creek Rancheria applies the policy of "Tribal and Indian Preference" when filling vacancies. Tribal and Indian Preference shall be given to qualified applicants who are enrolled members of a federally recognized Indian tribe or band. Ability to write accurate, grammatically correct, polished reports and business correspondence.Must be able to speak with the public professionally.MATHEMATICAL SKILLS:Must have basic math skills (ability to add, subtract, multiply, and divide).REASONING ABILITY:Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions furnished in written, oral, diagram, or schedule form. Ability to process detailed work and requests for information or assistance in a timely manner.Ability to apply regulatory and procedure doctrine, concepts, and policy to practical situations.LICENSES, CERTIFICATES, AND REGISTRATIONS:Must possess a valid California Driver's License with a clean driving record and be insurable under the Tribe's insurance policy. PHYSICAL DEMANDS:Must be able to sit or stand for extended periods.Must be able to lift and carry up to twenty-five (25) pounds.Must be able to respond to visual and aural cues.WORK ENVIRONMENT:The work environment characteristics described here represent those employee encounters while performing the essential functions of this job.
