Position Summary:

Under the direct supervision of the Assistant Executive Director or designee, and in collaboration with the Mental Health Services Team, the Facilitator acts as the coordinator for each youth assigned to them. This job role coordinates and leads the child and family team, providing effective and clear communication throughout the Program to ensure that all youth are receiving individualized, coordinated, family inclusive, strength-based services.


  • Responsible for the development of a Child and Family Team and ensures that the youth, residential staff, mental health services staff, family and/or caregivers, natural supports and placement support individuals are working collaboratively to meet the needs of all youth in accordance with the Treatment and Needs and Services Plans.
  • Acts as the primary liaison for all related parties and ensures consistent and comprehensive communication related to the youth's needs, including but not limited to placement workers, family members, and school agencies.
  • Provides consultation with the Residential Services Supervisors and other residential services staff in the maintenance of a highly structured, safe and secure residential environment that includes all appropriate individualized services for each youth assigned.
  • Reviews and investigates as indicated all assigned youth Special Incident Reports, ensures comprehensive communication to treatment teams regarding incidents, and facilitates service plan changes as indicated.
  • Facilitates Child and Family Team meeting.
  • Provides MediCal reimbursable services in accordance with Agency goals.

Essential Requirements:

  • A Master's degree from an accredited college or university and two years experience or a Bachelor's degree and four years of experience in a mental health setting as a specialist in the fields of physical restoration, social adjustment, or vocational adjustment. Up to two years of graduate professional education may be substituted for the experience requirement on a year-for-year basis. An Associate's degree from an accredited college or university and six years' experience may be considered with the approval of the Chief Executive Officer or the Chief Operating Officer. Note: Must meet State of California MediCal requirements as a Qualified Mental Health Professional.
  • Must possess a valid California driver's license, personal automobile insurance and driving record that meets the standards outlined in the Agency's Personnel Policy; Motor Vehicle Operating Standards.
  • Must be physically and mentally fit to work with potentially violent clients in accordance with the Agency's Physical Fitness Standards and Examinations Personnel Policy; must be willing to complete a health screening physical examination that includes drug and back screens; must have the unrestricted capacity to employ physical restraint techniques in a professionally safe manner.
  • Must be willing to complete a personal background investigation conducted by the State of California and must meet California Community Care Licensing standards for employment.

Hourly Range: $24.00 - $24.96
