Overview

The Family Partner is a formal member of the Kaiser Wraparound team whose role is to serve the family (particularly the caregiver and other natural supports), and help them engage and actively participate on the team, in order to make informed decisions that drive the Wraparound process. Family Partners have a strong connection to the community and are very knowledgeable about resources, services, and supports for caregivers. The Family Partner's personal experience as a consumer of mental health services is critical to building rapport and trust with caregivers, and advocating for their needs, when necessary. The Family Partner can, at times, be a mediator, facilitator, or bridge between families and agencies. The Family Partner ensures each family is heard and their individual needs are being addressed and met.

This program is a partnership between Kaiser Permanente and Seneca, providing behaviorally focused mental health services to children and families. These services, which are child and family focused and strength based, are provided in the office and community, including, but not limited to schools, hospitals, and family homes.

Responsibilities

* Demonstrate high level engagement and alignment skills, in order to build trust and rapport with caregivers and family members. * Provide guidance, expertise, and support for caregivers or any other natural supports. * Provide coaching, encouragement and support of caregivers and family members to use their own voice to express needs and concerns throughout the treatment process, in order to make informed choices for their family. * Use knowledge of local community resources, school systems or other mental health services to help caregivers and families identify and connect to a family safety network or natural support team. * Complete action steps identified by the treatment team, which may include support with transportation to medical or mental health appointments for caregiver or connection to basic needs resources in the community. * Work as a collaborative advocate, helping caregivers and family to understand the mandates and perspectives of other team members while navigating through county systems (child welfare, adoptions, probation, and school). * Deliver culturally competent and sensitive services by discovering what is important and unique to each family and tailoring service location, language or focus to those needs. * Make efforts to understand the history of Black, Indigenous, People of Color (BIPOC) in the United States and further Diversity, Equity, and Inclusion. * Model and utilize a strengths-based approach when in all interactions with family as well as in required documentation.

Qualifications

* Must have personal experience raising a child who has received services (Special Education, Social Services, Probation, Mental Health). * High school diploma/GED or higher; an Associate or Bachelor's degree preferred. * Experience working with families. * Own a vehicle, California driver's license, and auto insurance. * Clear TB test, fingerprints, and any other mandatory State/Federal licensing or certification requirements. * Ability to work as a team player and with a diverse population.

Schedule

* Full-time * Must have a flexible schedule with the ability to work evenings and weekend days
