DescriptionCCDSR participates in E-Verify, an Internet-based system that compares information entered by an employer from an employee's Form I-9, Employment Eligibility Verification, to records available to the U.S. Department of Homeland Security and the Social Security Administration to confirm employment eligibility. To learn more, please go to: SUMMARY: The Case Manager, Housing Stabilization II Project Hope (Bilingual) is a direct service position that provides housing-focused services to transition individuals and/or families from experiencing homelessness to housing. The Case Manager, Housing Stabilization II Project Hope (Bilingual) completes participant-centered assessments, creates individualized case plans, and facilitates mutual goal setting with the purpose of stabilizing housing resources/services to secure housing.Typical Duties Include (but are not limited to):Conduct screening and assessment with participants with the objective of determining housing barriers, and appropriate housing intervention.Divert, link, or schedule individuals with the appropriate housing-focused resources and/or services.Practice assertive engagement with individuals and families through persistent and creative activities to motivate, encourage participation, and promote follow-through.Provide team support to supervisor, shelter staff, and participant advocates to support participant's success.Maintain up-to-date and thorough Case Records and document service encounters as required, input data in a timely manner in Homeless Management Information System (HMIS), DataLink, and or Smartsheets.Respond to calls, emails, update requests and other inquiries in a professional and timely manner.Follow all best practice standards of care and CCDSR Policies and Procedures, including Release of Information, informed consent, confidentiality, and file documentation.Complete required trainings and certifications within the established time period.Complete more than one training and/or certification to be able to provide specialized services.Serve on agency and/or community service committee representing the department and/or organization.Conduct case management related duties independently, exercise sound professional and case management judgement, maintain Case Records with minimal documentation errors and/or corrective actions.Perform other related duties as assigned.Catholic Charities considers this position to be a mandated reporter of child/elder abuse.Agency Culture: It is essential that all employees of Catholic Charities aspire to the following:A commitment to the agency's mission, vision, and values;A commitment to excellence in everything we do;A commitment to accreditation as well as performance and quality improvement;A commitment to outcomes and measured results;A commitment to innovation and to what is possible.Education, Experience, and Skills Required:Fluency in English/Spanish is REQUIRED. Passing a CCDSR bilingual fluency test is a condition of employment.Associates Degree in a social service or related field, one to two-years' experience as a case manager in housing Stabilization or equal work experience or a bachelor's degree, required.Strong written and verbal communication, organizational, conflict resolution, and conflict resolution skills.Experience as an AOD counselor working with people with addiction issues is highly desirable.Computer literacy required, including experience with the Microsoft 365 suite.Understanding of causes of homelessness, experience working within social services and familiarity with community resources. Knowledge of Housing First principles and methods preferred.Cooperative, friendly, and helpful attitude with clients and co-workers. Ability to work closely with other employees to ensure positive, constructive environment within the program or department, and throughout the agency.Ability to work under pressure with a positive, solution-oriented approach.Passion and enthusiasm for the mission of CCDSR and its clients.Possess valid driver's license; at least state required minimum of auto insurance. Must be willing and able to travel/drive throughout Sonoma County.Must pass DOJ background clearance (fingerprinting) in accordance with the Diocese of Santa Rosa's policies, prior to start of employment.Physical Requirements:Requires ability to sit up to 3-6 hours per day.Requires ability to walk, stand, bend, squat, pull/push, and grasp up to 3 hours per day.Occasionally may be required to lift items up to 25 pounds to a height up to 3 feet.Occasionally may be required to carry items up to 25 pounds for distances up to one block.CATHOLIC CHARITIES IS AN EQUAL OPPORTUNITY EMPLOYER
