Job DetailsLevel EntryJob Location Santa Rosa Facility - Santa Rosa, CAPosition Type Full-Time/Part-TimeEducation Level High SchoolJob Shift AnyJob Category Health CareDescription POSITION PURPOSE: Performs the duties of a nursing aide as assigned by the Charge Nurses and/or Director of Nursing. Is accountable for all patient care given during shift. Responsible to adhere to all nursing care standards. ESSENTIAL SPECIFIC RESPONSIBILITIES: Oral hygiene: A.M. Care (bath-tub, shower, bed, etc.) P.M. Care (bath-tub, shower, bed) Sitz bath as ordered. Provide clean linen when appropriate: Make unoccupied bed Make occupied bed Change and straighten linen to remove wrinkles Give and remove bedpan/urinal. Assist patients to and from bathroom. Documentation: Perform ADL charting Bowel & bladder training and documentation Meal percentages charting Positioning patients: Position patients according to Charge Nurse's instructions. Changes patient's position appropriately and as frequently as instructed. Dietary needs: Ensure patient receives prescribed diet. Place tray conveniently for patient (on over bed table with head of bed up), remove covers from food, cut meats, etc.) Assist or feed patient as needed. Encourage good food intake to promote healing. Provide all snacks at ordered time (especially for diabetic patients, etc.) Remove tray and inform Charge Nurse of patient's appetite (good, fair, poor, or refused). Provide fluids as instructed. Intake and Output: Post I & O sheet in room and I & O sign on closet, near bed, or as instructed. Document on I & O sheet all intake and output for the shift (except IV and tube feeding), suction, T-tubes and Foley outputs. Turn I & O sheet in to Charge Nurse 30 minutes prior to end of shift. Specimen collection: The Nursing Aide will assist in obtaining specimens according to the Charge Nurse's instructions: Urine Sputum Stool 24-hour urine Clean catch urine Label all specimens with full name and time collected Vital signs: The Nursing Aide will take vital signs as instructed by Charge Nurses. Included are: Temperature as ordered (orally, rectally, or auxiliary) Heart rate (pulse) Respiration rate - including description (rapid, shallow, deep, regular, or moist, etc.) Blood pressure Assess pain scale The Nursing Aide will notify RN or LVN of any change or abnormal findings Vital signs will be recorded on wing sheet at times requested by Charge Nurse. Treatments: The Nursing Aide will be responsible for the following, for patients assigned to him/her, as instructed by Charge Nurses: Weigh patient Give perineal care Apply vaginal pads Apply ice cap Apply ace bandages Care of body past death Provide Foley catheter Apply slings under director of Charge Nurse Sponge baths under direction of Charge Nurse The Nursing Aide will assist RN or LVN with the following: Use of traction Draping, examination and treatment of patients Care of patient with: Oxygen mask or cannula Foley catheter and securing catheter bag to bed Colostomy Cast Admissions: The Nursing Aide will be responsible for the following: Bringing patient to and from room to other areas of facility Assisting patient to bed and check for patient armband and name plate on door Obtaining vital signs, height and weight, noting allergies, and reporting to RN or LVN Assuring patient is comfortable and oriented to surroundings Assisting RN or LVN with initial patient interview Perform C.N.A. admission checklist Issuing bath basin, bedpan/urinal and fresh water Patient/family orientation - instruct/explain the following: Call signal Bed controls Thermostat Telephone T.V. Visitor policy Mealtimes Bathroom, showers or bath Introduce roommate Smoking policy Side-rail policy Coffee/tea room for visitors Discharge: The Nursing Aide will be responsible for the following: Helping the patient collect personal belongings Taking patient per wheelchair to vehicle Removing linen from unit Returning equipment from unit to store room Transfers: Assist patient to wheelchair or stretcher Accompany patient to new room or vehicle Provide with call light and orient to new surroundings Report patient condition to Nursing Aide resuming care Pack and inventory belongings for routine transfers Comfort of patient: The Nursing Aide is responsible for providing the following: Patient privacy Skin care - keeps skin dry and clean, reports any noted reddening areas Straightening of patient's room (keep closets clean and organized) Answering call lights promptly Additional comfort measures Informing family or responsible party regarding clothing needs Equipment: The Nursing Aide should be familiar with the care and operation of the following: Care of orthopedic equipment Care of URC equipment (suction, gomco, K-pads, nebulizer machine, feeding tube, concentrators, etc.) Oxygen equipment - where to obtain to assist Charge Nurse Any equipment necessary in performing his/her duties Safety: Ensure beds are in low position and locked at all times Ensure that patients are secured appropriately when transporting by wheelchair or stretcher Assist patients in moving, sitting, or ambulating as necessary Ensure that needed articles are within patients' reach (water, tissues, bedpan, etc.) Ensure that side rails are in position as indicated Ensure that patient smoking policy is enforced Ensure that patients remain restrained when necessary Help to ensure that the patient rooms remain free of clutter (N) Ensure that the safety checklist is completed at the end of each shift Ensure that the resident assisted to the bathroom is not left unattended Help to ensure floors are dry - wipe up spills immediately and pick up objects which could cause patients to fall Ensure good lighting in halls and bathrooms Infection control: Use good hand washing, aseptic techniques and universal precautions Adhere to Isolation procedures Help to keep the unit clean (N) Gives good peri-care to patients daily Attends require in-service education programs, staff and unit meetings. Support and adhere to Nursing Service and Administrative policies and procedures. Utilize the aide check sheet as a means of reporting to the Charge Nurses. Attend Walking Report and is prompt to Change of Shift Report, if required. Cooperate and offer help to all personnel. Answer all pages immediately. Answer all call lights immediately. Relate all patient questions/requests to nurse in charge. Stay available and accessible at all times when on duty. Wear transfer belt. Utilize restorative principles in all aspects of patient care. NON-ESSENTIAL SPECIFIC RESPONSIBILITIES: Maintain Friends House standards of personal appearance and grooming. Must have a positive attitude, an attention to detail and enjoy interfacing with staff/residents. Know the Patient Bill of Rights and Non-Abuse policy. Support and embrace Friends House's Core Values (Respect, Compassion, Hope, Appreciation). Be knowledgeable of emergency procedures and act in a calm manner when there is an emergency. Other duties as assigned by Charge Nurse and/or Director of Nursing. Qualifications JOB SPECIFICATIONS: Hours: Vary according to schedule. Knowledge: Requires some knowledge of the nursing services field. (E) Must have Nursing Aide certification. (E) Must be state licensed at time of hire. (E) Prefer be CPR and first aid certified. (E) Independent Judgment: Works toward the accomplishment of patient care objectives as determined by the supervisor. Must use good judgment and common sense in regard to patient care. Questionable matters are referred to supervisor. Relations with Others: Job role requires substantive interaction with residents. Requires a high level of tact and judgment to maintain positive relationships. Safety All employees are responsible for complying with facility safety policies and procedures and shall take the necessary measures to ensure a safe environment for oneself, co-workers, patients/residents, and visitors. Working Conditions Moderate to heavy physical conditions. On feet most of the time.(E) Involves heavy lifting when transferring patients of over 50 lbs.(E) Involves walking, bending, stooping, twisting, reaching, carrying, pushing, pulling and handling.(E) Occasional working on irregular surfaces or heights.(N) Exposure to uncomfortable temperatures or odors.(E) HIPAA: Maximum Blood Borne Pathogens Job classification: Category I Impact of Actions: Actions affect the quality of care delivered to residents and influence the facilities ability to meet state and federal guidelines.
