**Relief/ 8 HR / Day Shift / NUHW Contract** It's a great time to join our Rehab Team as a Physical Therapist Assistant! **Summary:** The Physical Therapist Assistant works under the supervision of an PhysicalTherapist. The Physical Therapist Assistant is responsible for direct patientcare, family/caregiver training, documentation, and team communication for allassigned patients. In addition, the Physical Therapist Assistant performsdepartmental supportive tasks, as assigned. **Responsibilities:** + Performs physical therapyservices under the supervision of the physical therapist. + Reports changes in thepatient's condition to the physical therapist adheres to agency policiesand procedures for nursing, infection control, biohazardous wastemanagement, quality improvement activities, safety and risk managementpractices. + Teaches the patient/familyregarding active and passive exercises and the patient's total physicaltherapy program. + Accurately and thoroughlydocuments patient care observations, interventions, and evaluations;prepares and submits completed documentation of the home care visit withinthe established time parameters in agency policy. + Supports coordination of careactivities among all disciplines on the care by participating actively incase conferences, identifying additional services needed and dischargeplanning with multidisciplinary team and other resources to achievepatient outcomes. + Makes ongoing therapyre-assessments, evaluates patient progress, updates the plan of care basedon findings, and reports significant changes in the patient's condition tothe physician, appropriate team members and/or Clinical Manager. + Maintains patient schedulesbased on orders and authorizations within the Medicare week; submitscaregiver schedules timely and accurately; communicates scheduling changesdaily; meets productivity standards demonstrating effective timemanagement skills. **Minimum requirements:** Graduate from a 2-year college-level program approved by the American PhysicalTherapy Association and has achieved a passing grade on the proficiencyexamination given by the California State Physical Therapy Assistant ExaminingBoard.
