RN Admissions Coordination - Acute Rehab

Job Description:

The Acute Rehabilitation Liaison (ARL) is responsible for developing and maintaining census through targeted goals of the business plan and for developing referral relationships within Sonoma County and Northern California. The ARL is responsible for face-to-face community and hospital contacts with an emphasis on developing effective relationships with key referral sources. The ARL is responsible for following CMS guidelines for ensuring a complete preadmission screening process, including the management of a database that tracks statistical data for referrals, admissions, and follow-up. The ARL is responsible for providing acute rehabilitation education to referral sources, families, and physicians. The ARL is responsible for the execution of health care sales activities in support of acute rehabilitation services and admission criteria. The position works closely with ARU Medical Director, Unit Staff and Management.

Responsibilities:
Responds promptly to all internal and external referrals to insure patients meet criteria for admission to the unit.
Applies CMS IRF-PPS guidelines to insure patients meet criteria for admissions to the unit.
Makes scheduled and unscheduled calls and visits to internal and external referral sources; provides a weekly schedule of contacts and calls to referral sources.
Responsible for coordinating all aspects of the patient's admission to the unit.
With the assistance of informatics systems and personnel, develops and maintains patient database for referrals, admissions, and follow-up. Collaborates with all related services to establish methodology for collecting appropriate information for clinical and other outcome analysis of program performance.
Participates in monitoring and improving patient, nursing, ancillary services, and primary care physician satisfaction with program.

Liaisons with Acute Rehab team; works closely with ARU Care Management staff in coordination of admissions.
Collaborates with SRMH Marketing Department to implement sales activities that effectively market Acute Rehab Services,
Network with outside facilities who serve patients who would potentially benefit from the SRMH Acute Rehabilitation Program,
Responsible for gathering clinical information to conduct onsite or phone preadmission assessments according to CMS guidelines
Works with referral facility staff, internal and external care management staff, patients and families, and physicians towards timely referral processing and admissions
Completes pre-authorization requirements for all insured patients
Responsible for improving customer awareness of, access to, and satisfaction with acute rehabilitation services.
Acts as a resource and provides high-level service to all referral sources to facilitate timely admissions to the rehabilitation unit.
Other acute rehab related duties as assigned.
Responsible for driving to meet referral sources

Position Qualifications:
Bachelor's Degree preferred or equivalent job experience.
Minimum 2 years' experience as nurse, physical therapist, occupational therapist, medical social worker, speech pathologist, or tech in healthcare environment preferred.
Minimum of 1-year experience as nurse/clinical liaison or successful sales experience in healthcare environment preferred
Valid California Driver's License
