Job Description:

We are looking for a RN Liaison for the Hospice department at St Joseph Health!

Location: Memorial Hospice - Santa Rosa, CA

Schedule: Part time - 64 hours biweekly

Shift: 8-hour Day

Establishes and maintains ongoing relationship with case managers and other referral sources. Gathers all information needed for verifying coverage, making a business decision about accepting the referral, and scheduling the potential client for an assessment visit during the initial assessment. Recognizes both expressed and implied customer needs and recommend the solution that will most effectively meet the consumer---s desired outcome. Obtains the internal transfer of demographic and clinical information during intake so that transparent to the customer. Maintains awareness of the status of agency contracts and/or have a working knowledge of how to quickly access contract terms. Develops and maintains current information about services that exist in the marketplace and the availability of additional community resources that can be utilized to create complete customer service solutions. Shares accountability for achieving desired growth plans by educating referral sources about services, negotiating desirable rates that are within pre-approved pricing ranges and converting inquiries into referrals.

Initiates referral process in assigned facility after physician---s order is obtained and patient choice documented and coordinates the homecare referral process with the referrals source, hospital staff, physician, pharmacy, DME and other resources as needed.
Receives and completes potential home health referrals from assigned hospital, physician---s office and/or health care facilities to include complete physician---s orders for home health services, insurance information and complete, accurate demographics.
Provides ongoing education and consultation to hospital case managers, physicians and other referral sources regarding homecare services, patient potential needs and discharge planning options for homecare services.
Interviews and assesses patient---s medical, physical and psycho-social status to determine homecare needs, continuity of care needs, eligibility for services and appropriateness for homecare services based on coverage criteria and agency admission policy.
Collaborates with the physician, patient/family, hospital staff, outside resources and agency Intake Nurse and staff in developing a home health plan that meets the needs of the patient and promotes continuity of care to assure smooth transition for the patient from the hospital to the home.
Communicates daily with the home health office to support coordination of referrals, completeness of the referral information and medical documentation, and adequate planning for the admission process; Fax---s all pertinent medical information to intake in the homecare agency; pertinent payer sources, primary physician responsible for ordering homecare, and any additional information necessary to complete the referral process.
Provides information and education regarding homecare services and coverage criteria to the patients/families; addresses patient/family questions and concerns by explaining available services, explains the admission process and staff competency and availability.
Contributes to the home care education of the health care professionals, at assigned health care institutes by providing formal and informal training to staff, patients and their families.
Facilitates and/or coordinates interdisciplinary patient care by collaborating with all members of the health care team for gathering necessary case information and contributing to the development of an appropriate care plan for the patient; participate in case conference activities; provides clinical information to initiate the OASIS assessment and performs clinical interventions to support case set-up from the hospital to the home.
Completes, enters and/or tracks all referrals for submission to Intake Nurse; maintains record of daily inquiries, referrals, hospital holds and disposition to other facilities information for the Intake scorecard.

Additional Responsibilities:
Utilizes community resources as appropriate to meet patient needs.
Attends multidisciplinary patient care conferences as appropriate and staff meetings on a regular basis.
Participates in the development of intake policies, procedures, and protocols that adhere to Medicare, state and federal regulations, and credential criteria and standards.
Participates in the Corporate Compliance Program and Continuous Quality Improvement activities.
Must participate in Age-Specific training and other required training annually.
Must complete annual in-service on bloodborne pathogens.
May be asked to function as field staff or support administrative roles such as field nurse or social worker, or Intake Nurse as needed.
Completes other duties as requested or assigned by Executive Director and Director of Professional Services as needed.

Education: RN or MSW Required, if RN BSN preferred.
Experience: Three to five years--- experience in an acute care, home health or hospice environment.
License/Certification: RN or MSW; current CPR.

Preferred Position Qualifications: One to two years experience in home health or hospice, case management or discharge planning.
