RN Case Manager - Memorial Hospice

Job Description:

We are looking for a RN Case Manager with Santa Rosa Memorial Hospice, St Joseph Health!

Location: Hospice of Santa Rosa - Santa Rosa, CA

Schedule: Part time --- 64 hours biweekly

Shift: 8-hour Days

Summary:
Manages an assigned patient/family caseload from admission through the patient---s death. Develops and updates the patient/family Plan of Care working collaboratively with the members of IDT. Identifies patient/family care needs, implementing nursing interventions as appropriate. Evaluates the results of interventions and adjusts the Plan of Care accordingly. Demonstrates knowledge of costs related to patient care services and manages according to department guidelines. Communicates changes in patient/family condition to the Patient Care Coordinator and team members in a timely manner. Understands the details of insurance reimbursement for nursing services.

Responsibilities:
Demonstrates the ability to carry and manage a caseload of patient/family members and the integrity of the associated Plan of Care.
Demonstrates knowledge and skill in management of pain and symptoms including infusion therapies.
Demonstrates the ability to work as a team member and to coordinate care with IDT members.
Keeps the team informed about changes in the patient/family condition to facilitate teamwork in managing 24/7.
Demonstrates proficiency in the assessment, ongoing evaluation, coordination and timely/accurate documentation of the hospice patient care.
Demonstrates knowledge of the regulatory requirements for providing care to patients in all settings (e.g. home, SNF, Assisted Living) and the difference between these settings.
Demonstrates knowledge of costs related to care, hospice reimbursement and benefit program.
Demonstrates skill and proficiency in assessing appropriate use of therapies for enteral and parenteral nutrition.
Demonstrates competency in respiratory therapies including use of associated equipment.
Demonstrates sound understanding of and commitment to the hospice philosophy of team and care.
Enforces safety policies and procedures and safe practices.

Minimum Qualifications:
Education: Registered Nurse.

Experience: 3 --- 5 years--- experience as an RN. Hospice or Home Health experience preferred.

License/Certification: California Registered Nurse, CPR Certification.
