Transplant Coordinator (Contract) Location: San Francisco, CA Job
Type: 13-Week Contract with possible extension Start Date: April
20, 2026 Job Overview We are seeking an experienced Registered
Nurse – Transplant Coordinator to support their Living Donor
Transplant Program , with a primary focus on kidney and pancreas
transplants. This role involves coordinating patient care across
the full transplant journey, including evaluation, surgery, and
post-transplant follow-up. Schedule Shift: Day (5x8-hour shifts)
Hours: 8:00 AM – 4:30 PM Schedule: Monday–Friday On-Call: None
Weekends/Holidays: Not required Responsibilities Coordinate care
for transplant patients (pre, peri, and post-transplant) Monitor
patient progress and identify complications (infection, rejection,
etc.) Educate patients and families on medications, recovery, and
lifestyle changes Collaborate with multidisciplinary transplant
teams Manage transplant documentation and regulatory compliance
(UNOS, CMS) Track patient outcomes and maintain transplant
databases Assist with organ offers, allocation, and recipient
preparation Clinical Focus Living donor kidney and pancreas
transplants Pre- and post-transplant patient care Chronic
conditions such as ESRD, CKD, and transplant graft failure Work
Environment Acute care hospital and outpatient transplant clinic
Collaborative transplant team with interdisciplinary support May

Active California RN License (required at submission) BLS
Certification (AHA required) Minimum 2 years of RN experience
Experience with Epic charting system (preferred) Transplant
coordinator experience strongly preferred Preferred Qualifications
CCTC (Certified Clinical Transplant Coordinator) Experience in
transplant programs or acute care hospital settings Strong
knowledge of immunosuppressive therapy and transplant protocols
