Exciting Opportunity for Case Managers!/n/nJob Description:/n- Join our dynamic team as a Case Manager and make a real impact in the lives of patients!/n- Start Date: ASAP/n- Shift: Day shift, 5x8 hours (08:00 - 16:30)/n- Experience Required: 2 years of acute Hospital Case Manager experience in Care Coordination and Discharge Planning/n- Patient Ratio Experience: 1:20/n- Travel Experience Required: Yes (can be screened for experience requirements)/n /nKey Qualifications:/n- Must have experience in a fast-paced healthcare environment/n- Preferred experience in Teaching Hospitals, Community Hospitals, LTAC, Trauma Level I and II settings/n- Certifications:/n - BLS (preferred)/n /nEssential Skills:/n- Proficiency in charting systems, specifically Epic/n- Experience with:/n - Care coordination/n - Discharge planning/n - Needs assessment and DME orders/n - Prior authorizations/n - Knowledge of CPT, ICD 10 coding, and CMS guidelines/n /nWork Settings Experience:/n- Ambulatory/n- Behavioral Health/n- Home Health/n- Hospice/n- Acute Hospital settings/n/nSchedule Details:/n- Weekend Coverage: 2 weekends every 6 weeks/n- Pre-Approved Time Off: 1 request/n- Orientation Hours: 40/n- Patient Age Groups: Adults, Geriatrics/n/nCollaborative Environment:/n- Work alongside interdisciplinary support teams: IV Teams, Physical Therapy, Respiratory Services, Social Services, and more!/n- Participate in family meetings and treatment plan development./n /nSpecial Equipment:/n- Computerized documentation required./n/nFloat Policy:/n- Travelers may need to float between like units based on department needs./n/nWhy Catalytic Solutions (CatSol):/n- At CatSol, we connect dedicated therapists with rewarding assignments across the country.
