Hospice, Palliative Care, Skilled Nursing Facility, Long-Term Acute
Care, Behavioral Health, Sub-Acute, and Acute Rehabilitation
Description: The RN Case Manager is responsible for care
coordination, care transitions, and discharge planning throughout
the acute care patient experience. They will complete timely
initial assessments, updated notes, and final notes. They will
complete referrals to post-acute levels of care. The RN Case
Manager will coordinate with the multidisciplinary team to ensure
that the discharge is on track. Candidates should have experience
with appeals and denials, care coordination, needs assessment, plan
of care, and prior authorizations. Knowledge of CMS guidelines and
HIPAA regulations is essential. Floating to cover additional
campuses may be required if census allows. Candidates should be
prepared for a dynamic work environment that involves
