Registered Nurse (RN) | Case Manager Location: Santa Rosa, CA Agency: Lancesoft Inc Pay: $3,100 to $3,200 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position RN - Case Manager Requirements Years of Experience: 2 year(s) Note: 2-year acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment. Patient Ratio Experience: 1:20 Charting System: Required, Epic Certfications: BLS Weekends Required: 2 weekends every 6 weeks Call not required Will this traveler need to float between like units? Yes, based on need HARDSTOP, clinician must have current work history within 2 years. Special Procedures/Unit Details: • Ability to organize and participate in family meetings. • Participate in the development of the treatment plan. • Experience with HMOs, IPAs, etc. • Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently.
