Job Description About Hammock Homes Hammock Homes provides
supportive housing and care coordination for individuals rebuilding
their lives after homelessness, incarceration, hospitalization, and
substance use treatment. We partner with counties, hospitals, and
health plans to stabilize housing and improve health outcomes. This
role supports men residing in our Sonoma County supportive housing
program and includes both case management and Enhanced Care
Management (ECM) responsibilities, along with day-to-day house
accountability. Position Overview The Care Coordinator is
responsible for supporting male participants in our supportive
housing program through hands-on case management, care
coordination, and house oversight. This is an on-site, field-based
role that requires organization, follow-through, and comfort
enforcing rules and boundaries. The role involves daily engagement
with participants, coordination with partners, and maintaining
structure within the home. Key Responsibilities Serve as a primary
point of contact for men residing in the home Provide ongoing case
management and care coordination Support participants with housing
navigation, appointments, benefits, ID replacement, employment
readiness, and life skills Support enrollment and participation in
Enhanced Care Management (ECM) services Assist with care plans,
coordination, documentation, and follow-up Enforce program rules,
expectations, and house policies consistently Conduct and document
urinalysis (UA) testing as required Monitor participant compliance,
engagement, and overall stability Support and enforce chores,
cleanliness, and shared-space standards Respond to conflicts,
de-escalate situations, and enforce boundaries Document incidents
and escalate concerns according to protocol Assist with grocery
planning, purchasing, and supply management Monitor home conditions
and report maintenance needs Maintain accurate, timely case notes
and required documentation Communicate with house managers,
leadership, ECM partners, and referral sources Participate in team
meetings and case conferences Qualifications (Required) Experience
in case management, care coordination, social services, behavioral
health, addiction treatment, or housing programs Comfortable
enforcing rules and holding boundaries while remaining respectful
Experience working with justice-involved individuals and/or people
in recovery Strong organization, follow-through, and documentation
skills Reliable transportation and ability to travel locally
Professional verbal and written communication skills Preferred
Qualifications (not required) Experience with Enhanced Care
Management (ECM) or Medi-Cal–funded programs Background in
supportive housing, sober living, or homelessness services
Familiarity with county systems, benefits programs, or managed care
plans Bilingual (Spanish/English) a plus Compensation & Benefits
$27 – $30 per hour starting , depending on experience Opportunity
