Responsibilities: Coordinate and facilitate care transitions
throughout the hospital stay and across the care continuum
Collaborate with patients, families, physicians, and
multidisciplinary teams to develop and implement care plans Assist

when assigned Requirements: Active California RN License Minimum 2
years case management experience in acute care Experience with
