Job Description:

*Job Summary:*The RN Case Manager is responsible for providing comprehensive case management services, including assessment, planning, implementation, coordination, monitoring, and evaluation of patient care. The goal is to ensure efficient, high-quality care delivery and optimal outcomes for patients across the continuum of care.*Key Responsibilities:** Coordinate care and services for patients across various levels of care* Collaborate with physicians, nurses, social workers, and other health care providers* Develop individualized patient care plans based on clinical assessments and discharge needs* Facilitate appropriate use of medical resources and advocate for patient needs* Monitor patient progress and modify care plans as needed* Ensure compliance with regulatory and organizational policies* Educate patients and families on care plans, discharge instructions, and community resources* Participate in interdisciplinary team meetings and case reviews*Requirements:** *Active California RN license ** *Minimum of 2 years of RN Case Management experience** *Current and active BLS certification** Strong clinical assessment and critical thinking skills* Excellent communication and organizational abilities* Familiarity with care transitions, discharge planning, and utilization review*RN01*Job Types: Contract, TemporaryPay: $72.00 - $77.00 per hourExpected hours: 40 per weekSchedule:* 8 hour shift* Day shift* Weekends as neededAbility to Commute:* Santa Rosa, CA 95405 (Required)Ability to Relocate:* Santa Rosa, CA 95405: Relocate before starting work (Required)Willingness to travel:* 25% (Preferred)Work Location: In personby Jobble
