Job Description:
Social Worker - Palliative Care Direct
Location: Santa Rosa, CA
Work Schedule: Part Time - 48 Biweekly Hours
Shift: 8-hour, Days
Provides psychosocial assessment and delivery of counseling, social
work and emotional support services to terminally-ill patients,
and/or palliative services to patients and their families. Works as
a team member to identify problems and assist patient/family
members in exploring realistic options, decision making and problem
solving.
