What Health System Pharmacy contributes to Cardinal Health
Health System Pharmacy is responsible for providing customized
pharmacy program solutions that reduce costs and improve patient
care quality for hospitals, health systems and other integrated
healthcare providers may also consult with and advise healthcare
team on prescribed medications, supplies and related processes.
Community Health Center Pharmacist
Cardinal Health Community Health Center programs are designed and

Counsel patients on drug usage and educates health care personnel
on drug utilization.
Interview and examine patients for drug evaluation; checks on drug
adherence and side-effects; discusses symptoms and effects of
prescribed drugs with patients and maintains a drug history on
Consult with clinicians on drug usage and the drug regimen of
specific patients particularly hypertensive, diabetic, and
psychiatric patients; recommends changes in the drug or the dosage
prescribed for these patients.
Given a diagnosis, synthesize a treatment plan, which may include
drug therapy and lifestyle modifications, and transition within the
continuum of care settings.
Identify, resolve, and prevent potential and actual
medication-related problems including untreated indications,
improper drug selection, subtherapeutic dosage, overdose, failure
to receive medication, adverse drug reactions, drug interactions,
and medication without indication.
Conduct both formal and informal training for nursing staff,
medical residents and other health care staff on drug utilization,
dosage, incompatibilities, preferred dosage forms, evidence-based
medication therapy and toxicity, etc.
Perform drug utilization review, comprehensive medication review,
and/or medication reconciliation.
Document information appropriately in the patient's medical record
and other computer systems according to policies and procedures and
Maintain expertise on the 340B Drug Pricing Program
Oversee activities of 340B Coordinator
Perform other related duties as assigned
Comprehensive knowledge of current
pharmacological/biopharmaceutical principles, medical terminology,
pathologies, disease state management, age-appropriate therapy, and
other information as it pertains to medical or pharmaceutical care
Knowledge of Federal/State laws/regulations regarding delivery of
pharmacy services and the scope of practice of a pharmacist.
Knowledge of sources of current drug information and medical
Demonstrated ability to extract pertinent information from
patient/other sources for a medical management plan.
Demonstrated ability to teach and evaluate.
Effective verbal/written communication skills at appropriate
Bachelors degree in pharmacy required ,PharmD preferred
Valid Registered Pharmacist license issued by California Board of
One (1) of the following:
Completion of six (6) months of a pharmacy residency program;
Two (2) years of professional level experience as a Clinical
Possession of a Board Certification in a pharmacy specialty
A Covid-19 vaccination or a legitimate accommodation is required in

Bilingual fluency in English and Spanish highly desired.
