What Health System Pharmacy contributes to Cardinal Health

Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.

Community Health Center Pharmacist

What is expected of you and others at this level

Applies comprehensive knowledge and a thorough understanding of concepts, principles, and technical capabilities to perform varied tasks and projects

May contribute to the development of policies, procedures, and guidelines

Works on complex projects of large scope

Develops technical solutions to a wide range of difficult problems. Solutions are innovative and consistent with organization objectives

Completes work independently receives general guidance on new projects

Work reviewed for purpose of meeting objectives

May act as a mentor to less experienced colleagues

Report at least monthly to a designee of Customer's choosing on the senior leadership team.

Counsel patients on drug usage and educates health care personnel on drug utilization.

Interview and examine patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.

Consult with clinicians on drug usage and the drug regimen of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.

Given a diagnosis, synthesize a treatment plan, which may include drug therapy and lifestyle modifications, and transition within the continuum of care settings.

Identify, resolve, and prevent potential and actual medication-related problems including untreated indications, improper drug selection, subtherapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.

Conduct both formal and informal training for nursing staff, medical residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.

Perform drug utilization review, comprehensive medication review, and/or medication reconciliation.

Document information appropriately in the patient's medical record and other computer systems according to policies and procedures and regulatory requirements

Maintain expertise on the 340B Drug Pricing Program

Oversee activities of 340B Coordinator

Perform other related duties as assigned

Responsibilities

Comprehensive knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathologies, disease state management, age-appropriate therapy, and other information as it pertains to medical or pharmaceutical care management plan.

Knowledge of Federal/State laws/regulations regarding delivery of pharmacy services and the scope of practice of a pharmacist.

Knowledge of sources of current drug information and medical literature.

Demonstrated ability to extract pertinent information from patient/other sources for a medical management plan.

Demonstrated ability to teach and evaluate.

Effective verbal/written communication skills at appropriate level.

Qualifications

Bachelors degree in pharmacy required ,PharmD preferred

Valid Registered Pharmacist license issued by California Board of Pharmacy

One (1) of the following:
Completion of six (6) months of a pharmacy residency program; OR

Two (2) years of professional level experience as a Clinical Pharmacist; OR

Possession of a Board Certification in a pharmacy specialty area

A Covid-19 vaccination or a legitimate accommodation is required in order to be employed in this position. Covid-19 vaccination includes:

2 doses of the Moderna or Pfizer vaccine / 1 dose of the Johnson & Johnson vaccine

1 booster dose of the vaccine (Johnson & Johnson, Moderna, or Pfizer)

Preferred Qualifications

Completion of an ASHP-accredited pharmacy residency and/or specialty residency in geriatric pharmacy or psychiatry; OR two years of full-time professional level experience as a clinical pharmacist in this setting.

Bilingual fluency in English and Spanish highly desired.

Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.

Clinical experience working in substance use disorder or mental health settings.

Possession of a valid Advanced Practice Pharmacist license in California.
