Job Description:
Description We are looking for a Speech Language Pathologist for
the Speech Therapy Department at Sonoma County Entities. Location:
Healdsburg, CA Work Schedule: On Call / Per Diem Shift: 8-hour,
Days Job Summary: The practice of Speech Language Pathology shall
include assessment, treatment planning and implementation,
education, communication and demonstration of leadership to address
the needs of patients, their families and caregivers as well as to
promote and demonstrate a positive working relationship among
professionals and ancillary staff. Competencies in line with our

and stewardship. Essential Functions:
- Able to comprehensively assess the functional needs of the
patient and sets appropriate goals for each patient. Provides
direct patient care, evaluates outcomes, consults with other
specialists as needed. Provides reassessment of patient's
functional needs and adjusts care plan as indicated to ensure
optimal patient outcome.
- Demonstrates understanding of Medi-Care guidelines and
regulations relating to medical necessity, sustained functional
improvement, reasonable frequency and duration of treatments and
procedures for documentation.
- Establishes measurable short and long term goals within
reasonable time frames. Consistently works to achieve functional
goals by selecting appropriate therapeutic methods and shows this
progression toward goals in daily documentation. Educates patients,
family and caregivers concerning diagnosis, goals and individual
home programs.
- Demonstrates professional, appropriate verbal and written
communication skills with patients' treating physician and other
associated personnel.
- Adheres to rules regarding standards of best practice and
ethics of regulatory agencies. Complies with current licensure laws
and rules related to aides and assistants. Assures that licensure
requirements for annual renewal are met in a timely manner.
- Adheres to established hospital and departmental policies and
procedures, core values, quality and improvement programs, and
safety, environmental and infection control standards.
- Maintains patients' medical record, and any other patient
related documents, accurately, completely, neatly and
confidentially, in a timely manner.
- Provides direction and leadership to support staff, new hires,
students and new graduates.
- Promotes professional growth and development, personally and
throughout the department and organization with educational
programs, current research, in-service meetings and workshops.
- Demonstrates competence in fit and function of assistive and
orthotic devices.
- Performs other duties as assigned to support the department's
objectives. Skills:

- Knowledge of practice and principles of Physical Therapy.

- Ability to seek and gain new knowledge and abilities.
- Ability to assess the functional needs of the patient and set
appropriate goals.
- Understanding of Medi-Care guidelines and regulations. Minimum
Position Requirements: Education: Coursework/Training - Graduation
from accredited speech language pathology program.
Licenses/Certifications:
- California License or Eligibility.
- Current BLS.
- Current CPR certification. Preferred Position Qualifications:
Experience: 2 years experience as a licensed Speech language
Pathologist. However, outstanding new graduates considered. About
