Description Social Worker Location: Healdsburg, CA Work Schedule: Full Time - 80 Biweekly Hours Shift: 8-hour, Days Responsible for bio-psychosocial assessment of patients in the Sub-Acute, Medical/Surgical and all other areas of the Hospital in which patient care is provided. Provides counseling supports for end of life concerns, addictions and other psycho-social issues both short and long range planning. Required Qualifications:

• Master's Degree in Social Work, Marriage and Family Therapy, or Licensed Professional Clinical Counselor (LPCC), or related field or any combination of education and experience that would provide the required knowledge, skills and abilities.
• BLS Preferred Qualifications:
  • 3 years experience working in social services in an acute care setting/skilled nursing setting.
