Description:Alliance Medical CenterJob DescriptionJob Title: Behavioral Health CounselorDepartment: MedicalReports To: Behavioral Health DirectorPrepared By: HR AdministratorApproved By: Director of HREdit Date: 01/30/19_________________________________________________________________________________Summary The Behavioral Health Counselor at AMC works together with medical providers to integrate behavioral health and primary care for our patients. Provides culturally and linguistically competent services for our community. The Behavioral Health Counselor works with patients who have a range of emotional and behavioral health issues. Support patients who may be experiencing life transitions, depression, anxiety, or need support in reaching health goals.*** This position is onsite in our Healdsburg office.Essential Duties and Responsibilities:The Behavioral Health Counselor(BHC) will provide same day mental health "touches" or warm hand-offs to begin the process of engaging with patients to better support their health and wellness, as well as short-term counseling; group counseling; co-facilitating group medical visits with a PCP; care coordination; and case management services.The BHC will also work with the Medication Assisted Treatment team and provide support and counseling for those identified as having an Opiate Use Disorder and other Substance Use Disorders. The BHC will also screen for the need for referral to more intensive substance abuse services as needed.

  • Ability to work on a multidisciplinary team in a primary care setting
  • Knowledge of the principles, procedures, methods and techniques of behavioral health service provision, including:
  • Legal and ethical issues
  • Involuntary hospitalization/ 5150 process
  • CPS & APS reporting requirements
  • Danger to self and Danger to Others reporting requirements
  • Crisis resolution skills
  • Brief and solution focused therapy
  • Substance abuse disorder diagnosis and treatment
  • Ability to assess, diagnose and treatment plan according to Medi-Cal guidelines with proper documentation in an electronic medical record.
  • Experience charting in an electronic medical record.Supervisory ResponsibilitiesThis job has no supervisory responsibilities.. MFT, social work, psychology, community mental health)Language SkillsBilingual in English and Spanish, both written and verbal preferred. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence.Mathematical SkillsAbility to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.Reasoning AbilityAbility to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.Computer SkillsTo perform this job successfully, an individual should have knowledge of Database software; Internet software; Spreadsheet software and Word Processing software.Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
