Community Support Network is a growing non-profit organization fighting homelessness in Sonoma County. Generous health benefits Generous time off (vacation, holiday, and sick) Compensation : $17.00/ hour plus a value of between $2.78 - $7.80 / hour for company paid medical coverage. Essential Functions : Duties include but are not limited to providing assessment and teaching of independent living skills, individual and group supportive counseling, advocacy and referrals to support services as required; implementing individual service plans and contracts with clients ;providing on-going supervision and clinical evaluation of residents in program ;interviewing and assessing potential residents; administering intake and discharge procedures ;providing crisis intervention and crisis management services when necessary; maintaining case notes on individuals, as per Medi-Cal regulations; serving as HMIS coordinator and maintaining all guidelines and policies of Sonoma County HMIS (Homeless Management Information System). Minimum Education/Requirements : H.S. degree/GED required. BA preferred but will consider AA and other life experience and skills; HMIS training Certificate (or actively in training); ability to meet all licensing requirements; minimum one year experience in mental health field; valid driver's license and reliable transportation; excellent observation and listening skills; ability to focus and thrive in an ever-changing work environment.
